Introduction
Chronic obstructive pulmonary disease (COPD) is a preventable and treatable disease with some significant extrapulmonary effects that may contribute to the severity in individual patients. Its pulmonary component is characterized by airflow limitation that is not fully reversible.
COPD is a leading cause of morbidity and mortality worldwide, and results in an economic and social burden that is both substantial and increasing. It is projected that COPD, which ranked sixth as the cause of death in 1990, will become the third leading cause of death worldwide by 2020. Recent estimates suggest that up to 12 million Americans have a diagnosis of COPD, and there may be up to an additional 12 million with unrecognized airflow obstruction.
Target Audience
This activity was developed for Family Physicians, Nurse Practitioners, and Physician Assistants who treat patients with COPD.
Learning Objectives
Following completion of this case simulation, participants should be able to:
- Recognize and appropriately manage an acute exacerbation of COPD.
- Recognize that spirometry is required to make a diagnosis of COPD, and is also used to stage the severity and as a guide for therapy.
- Manage stable COPD, including the indications for long-term oxygen therapy, pulmonary rehabilitation, inhaled long-acting bronchodilators, and inhaled corticosteroids.
- Know when to refer a patient with COPD to a pulmonologist for consideration of other treatment options, such as lung volume reduction surgery.
